PURPOSE: The Advanced Practice Provider (APP) delivers
high-quality, patient-centered healthcare in a same-day access
primary care setting. This role includes assessing, diagnosing, and
treating patients across the lifespan with acute, chronic, and
preventive needs that require non-emergent same-day access. The APP
works independently and in collaboration with the care team,
aligning clinical practice with organizational values, goals,
compliance and performance standards. ESSENTIAL FUNCTIONS: Patient
Care Delivery Provide timely evaluation and management of patients
presenting for same-day care (walk-ins, urgent appointments, acute
issues). Conducts comprehensive history taking, physical
examinations, and diagnostic assessments to identify health
concerns, including urgent and same-day issues. Provides
evidence-based care across the lifespan (pediatric, adult, and
geriatric population). Demonstrate flexibility in adapting to a
high-volume, fast paced environment with unpredictable patient flow
with varying complexity. Develops and implements therapeutic
treatment plans within scope of practice, considering patients
physical, emotional, cultural, and financial factors. Orders and
interprets appropriate diagnostic tests; initiates timely follow-up
care and referrals based on results. Prescribes and administers
medications, immunizations, treatments, and minor procedures in
accordance with licensure, privileges, and clinic protocols.
Applies sound clinical judgment in managing acute, chronic, and
emergent medical conditions, escalating care when appropriate.
Clinical Documentation and Coordination Maintains complete,
accurate, and timely documentation of patient encounters in the
electronic health record in compliance with organizational and
regulatory standards. Coordinates care across disciplines, ensuring
consistent and effective communication among providers, nurses, and
care teams. Oversees assigned care team workflows to ensure
coordinated and efficient delivery of same-day care. Patient
Education and Advocacy Provides education and counseling to
patients and families regarding diagnoses, treatment plans,
medications, lifestyle modifications, and preventative care.
Support patient-centered shared decision-making. Identifies and
connects patients with community resources and support services as
appropriate for their health and wellness goals. Responds promptly
to patient inquiries, refill requests, and follow-up needs,
maintaining continuity of care and patient satisfaction.
Professional Practice and Compliance Practices in accordance with
state and federal laws, professional licensure regulations, and
organizational policies. Demonstrates commitment to patient privacy
and confidentiality in compliance with HIPAA and related
regulations. Follows established infection control protocols,
including standard precautions and bloodborne pathogen exposure
control plans. Organizational Representation and Values

clinical decisions and improve outcomes. Leadership and Teamwork
Supports and guides clinical staff within the care team, fostering
a collaborative, respectful, and effective team environment. Works
independently and collaboratively to meet patient needs, clinical
goals, and departmental expectations. Assumes additional
responsibilities as requested, demonstrating initiative and
flexibility to meet team and organizational needs. Professional
Development Maintains current licensure and certification;
completes all required training, education, and compliance modules
on time. Participates in continuing medical education (CME)
activities to maintain clinical competence and enhance professional
growth. Builds and maintains positive relationships with internal
departments and external healthcare partners. QUALIFICATIONS:
Current, unrestricted state licensure as NP or PA DEA license and
prescriptive authority in accordance with state law Certification
by a nationally recognized accrediting body (e.g. AANP, ANCC,
NCCPA) Current BLS or ACLS Minimum of 2 years of experience in
primary care, emergency care, or urgent care setting with patients
across the full lifespan. EDUCATIONAL REQUIREMENTS: Graduate of an
accredited Nurse Practitioner or Physician Assistant program Aviva
