Health Licensed Practical Nurse (LPN) plays a critical role in
delivering compassionate and skilled nursing care to patients in
their homes, primarily focusing on those requiring palliative care.
This position ensures that patients receive personalized care
tailored to their unique health needs while promoting comfort and
quality of life. The LPN will collaborate closely with
interdisciplinary healthcare teams to implement care plans, monitor
patient progress, and adjust treatments as necessary. This role
demands flexibility and responsiveness to varying patient schedules
and conditions, providing care on an as-needed basis. Ultimately,
the LPN contributes to the holistic well-being of patients by
combining clinical expertise with empathy and patient-centered
communication in a home health setting across the United States.
Minimum Qualifications: Current and valid Licensed Practical Nurse
(LPN) license in the state of practice within the United States.
Demonstrated experience or training in palliative care nursing.
Basic Life Support (BLS) certification. Ability to work
independently in a home health environment with minimal
supervision. Strong communication skills to effectively interact
with patients, families, and healthcare team members. Preferred
Qualifications: Previous experience working in home health or
hospice care settings. Certification in Palliative Care or Hospice
Nursing. Home Health experience Experience with electronic health
records (EHR) and documentation systems. Additional certifications
such as IV therapy or wound care specialty. Familiarity with
community resources and patient advocacy. Responsibilities: Provide
skilled nursing care to patients in their homes, including
administering medications, wound care, and monitoring vital signs.
Deliver palliative care services that focus on symptom management,
pain relief, and emotional support for patients with serious
illnesses. Collaborate with physicians, registered nurses, social
workers, and other healthcare professionals to develop and update
individualized care plans. Educate patients and their families on
disease management, medication administration, and health
maintenance to promote independence and well-being. Document all
nursing interventions, patient responses, and changes in condition
accurately and timely in compliance with regulatory standards.
Skills: The required skills of palliative care and empathy are
essential in delivering patient-centered care that addresses both
physical symptoms and emotional needs. Palliative care expertise
enables the nurse to manage complex symptoms effectively, ensuring
patient comfort and dignity. Empathy allows the nurse to build
trust and rapport with patients and their families, facilitating
open communication and emotional support during challenging times.
Preferred skills such as experience with electronic health records
enhance the nurse's ability to document care accurately and
coordinate with the healthcare team efficiently. Additionally,
certifications in specialized areas like wound care or IV therapy
expand the nurse's clinical capabilities, allowing for
comprehensive care delivery in diverse home health scenarios.
