the Position TravelNurseSource is working with Cynet Health to find
a qualified Home Health RN in Salem, Oregon, 97301! Job Title:
Registered Nurse Profession: Nursing Specialty: Home Health
Duration: 13 Weeks Shift: Day Hours per Shift: 8 Experience:
Minimum 1 full year of Adult Home Health Experience or 1 full year
of Hospice Experience depending on the specialty License: Must have
license in hand Certifications: CPR, BLS Must-Have: Proficiency in
OASIS and HomeCare HomeBase EMR within the last 6 months
Description: Provides coordinated and directed home nursing care
according to physician's orders. Follows agency policies and
procedures through the competent application of the nursing
process. Responsible to the Clinical Manager. Directly supervises
and evaluates Home Health Aides and Licensed Practical
Nurses/Licensed Vocational Nurses. Requires reliable transportation
and a current driver's license. Must have agency-required liability
insurance. Average number of patients per day ranges from 5 to 8.
Each type of visit and activity holds a different point value
towards weekly productivity expectations. Guaranteed hours are not
provided. Traveling is required up to 75 miles from the home
clinic. The contractor will be compensated for mileage at the IRS
standard rate. Travelers must accept every patient on their
schedule regardless of distance. Documentation must be completed
within 24 hours of each appointment with the patient. Delayed and
incomplete documentation that impacts follow-up patient care may
result in a cancellation. There are no guaranteed hours with this
contract, and weekly productivity points of 30 must be achieved.
Flexible to on-call rotations as needed. Candidates with recent
