TeamworkDEPARTMENTAL SUMMARY The Rehabilitation Department provides
therapy and nursing services to patients, families and patients
caregivers including: Rehabilitation Nursing Care, Physical
Therapy, Occupational Therapy and Speech Language Pathology. Our
department operates as a system department and provides services to
Acute Care, Inpatient Rehabilitation Facility and Outpatient
settings across four campuses. Our Rehabilitation departments offer
a variety of specialty services across the various services
delivery areas.POSITION OVERVIEW: The Physical Therapist evaluates,
plans, and provides treatment and rehabilitation as prescribed by a
referring physician. Works with family and team members regarding
patients abilities and goals.This position does not directly manage
any other caregivers.ESSENTIAL FUNCTIONS AND DUTIES:Adheres to
Oregon State Physical Therapy Licensing Board.Provides physical
therapy evaluation and treatment to achieve desired
outcomes.Provides staff consultation and training.Provides
community resource knowledge and referral services.Provides
physical therapy services to other hospital service areas.May be
required to drive Rehabilitation van and meet all related SCHS
vehicle policy requirements.Functions as a member of the
Interdisciplinary Care Team to assure patient care is accomplished
effectively and efficiently in a cost effective manner.Participates
in clinical care conferencing.Gathers data and accurately documents
information in a timely manner demonstrating compliance with
departmental and professional regulations.Participates with team
members in facilitating patients and familys learning throughout
continuum of care. Reinforces patients continued health care
through teaching and/or referral to community agency
follow-up.Supports the vision, mission and values of the
organization in all respects.Supports Lean principles of continuous

of change.Provides and maintains a safe environment for caregivers,

assigned.EDUCATION:Required: Bachelors, Masters, or Doctorate
degree in Physical Therapy from a college or university accredited
by the American Physical Therapy Association.Preferred:
N/ALICENSURE/CERTIFICATION/REGISTRATION:Required: Must have
necessary requisites for licensure as a Physical Therapist by the
Oregon State Board of Health. Valid Oregon driver's license and
ability to meet St. Charles Health System driving requirements.
Ability to travel to business functions/trainings/meetings and all
St. Charles Health System worksites. Current AHA Basic Life Support
for Healthcare Provider certification.Preferred: Preferably a
member in good standing of the American Physical Therapy

certification list.EXPERIENCE:Required: Must have one year
experience with therapy procedures, current orthopedic and
neurological follow-up procedures, and rehabilitation
practices.Preferred: Previous physical therapy experience in
hospital-based setting.PERSONAL PROTECTIVE EQUIPMENT:Must be able
to wear appropriate Personal Protective Equipment (PPE) required to
