Director Case Management - Registered Nurse (RN)Full-Time Position$20,000 Sign-On Bonus and Generous Relocation Package Available!!!Job Summary:Oversee a team of case managers responsible for patient care coordinationDevelop and implement case management programs, including utilization review, intake or discharge planning, and managed care contracting or negotiationEvaluate patient care data to ensure that care is provided in accordance with clinical guidelines and organizational standardsSeek treatment that balance clinical and financial concerns with the familial needs and patients' quality of lifeRely on extensive experience and judgment to plan and accomplish goalsLead and direct the teamA wide degree of creativity and latitude is requiredTypically reports to Senior LeadershipQualifications:Bachelor's degree in nursing or related field required, Master's degree in nursing or related field preferredCurrent licensure as a Registered Nurse in the State of OregonCurrent BLSRequires 3+ years of case management experience in an acute care settingRequires 5+ years of clinical experience an acute care settingExperience in process improvement and program developmentDemonstrated competency in teachingSupervisory experience preferredInterprets standards of practice, benchmarks of care as related to care management in the acute care settingExperience as a clinical preceptorManages specific populations as well as case management of individual patientsEffectively facilitates multidisciplinary teams and small groupsThis job description is intended to describe the general nature and level of work being performed.
